ASSUMPTION OF RISK, WAIVER, RELEASE, AND HOLD HARMLESS AGREEMENT

I am the parent and/or legally authorized guardian of the child participating in the St. Anthony Summer School 2017. On behalf of myself, my spouse, my child/ward (the "Participant"), and my/our heirs, personal representatives, and assigns, I understand and agree to the following:

I hereby permit my child/ward (the "Student"), to participate in St. Anthony High School (SAHS) Summer School, including all related travel, events and activities.

I will direct the Student to follow all instructions and guidelines provided by SAHS faculty and staff during Summer School.

In allowing the Participant to participate in the Summer School, I agree not to sue and I hereby release, waive, discharge, hold harmless, indemnify, and defend SAHS and the Archdiocese of Los Angeles, as well as their respective employees, volunteers, directors, affiliates, and representatives, from any and all liability, losses, damages, claims, actions, and causes of action of every nature for any and all known or unknown, foreseen or unforeseen, bodily or personal injuries, property damage, or other loss relating in any way to the Participant’s involvement in Summer School.

I am unaware of any health issue or restriction that would affect the Student’s participation in the Summer School. In the event of an emergency, I authorize SAHS to act for me in its best judgment in rendering any medical attention to the Student. I further understand that I will be responsible for any medical expenses relating to the Participant’s involvement in Summer School.

I understand that, as part of my child/ward’s participation in Summer School, SAHS may take photographs, film footage, and recordings through other media, some of which may contain the image, likeness or voice of my child/ward. I grant permission to SAHS to make this media and to use it for its educational and advertising purposes without financial compensation to me or my child/ward.

I have read and understood both the Regular Parent/Student Handbook and the Summer School Handbook. Both the student and I (parent) agree to adhere to the policies and procedures of both handbooks at all times.

I understand that the main mode of communication between instructor, student and parents will be e-mail. I (parent) have a working e-mail that I will check regularly for school communications during the Summer School session.

Paying for Summer School will be done through the FACTS online payment system. Once registration is complete you will receive a bill from FACTS for the full amount of Summer School. Payment in full is due by July 5th before financial suspension will take place from Summer School. Please make sure that you log onto FACTS to start making your payments. The sooner you register the longer a time period you have to make your payments.
